Use this form to register your organization's pick-up site for the Ride United/DoorDash program brought to you by United Way and 211. Please note that this service is completely FREE to you and the clients you serve.

This should be the actual name of the Pick-Up Site, not the overall Local Partner Agency Name. This Pick-Up Site name should be the name of the location that the Dasher would see upon arrival should the location be approved.

Enter the Pick-Up Site's unit or suite number -- if no unit or suite number exists, leave blank.
